I understand that Reiki is an energy technique that is used for stress reduction, relaxation, and healing. I understand that Reiki practitioners do not diagnose conditions. I understand that Reiki practitioners do not prescribe or perform medical treatment, nor interfere with the treatment of a licensed medical professional. I understand that Reiki healing energy and/or any recommendations I may receive during my session shall not replace medical or psychological care. I understand that all recommendations for healing are followed at my own choosing and risk. I understand that it is recommended that I see a licensed physician or licensed health care professional for any physical or psychological condition I may have.

I understand that the energy exchange for the receipt of Reiki healing is in the form of monetary payment and I agree to pay for services prior to my appointment.
